Equipment checkout and responsibility form for helmets, pads, jerseys, and other team-issued gear. Players are responsible for all checked-out equipment.

This form is given to the players the day they receive their equipment from school and will include the items that the player is receiving from the team. The form will be kept by the football head coach.
